There are probably more details, like exactly what the mosaic layout issue was, what the undo was (the admin was not experienced in the nuances of zmi undo) and if the admin and user were simultaneously working to resolve the issue.
It should not be important what has been done before the "undo".
The "undo" should either fail (and leave the state as is) or
restore to a consistent state. Apparently, "undo" failed to behave
like this. This might be an "undo" or relstorage bug.
